Here is what we (me and my chat gpt) landed on for the form. I left the notes in there so you guys can see. My chat is very trained on the Park Lex 60 tone, voice and patient base so I feel like this is pretty good. It won’t look like this exactly because it will be hosted by and outside form but I can embed it on the site so no one knows.
